To apply for PVG please register with us; however, we need a formal agreement to act on your behalf. You will need to read the Code of Practice and accept this as part of the Elite Childcare Agency contract terms along with the secure handling of disclosure information.

As soon as we receive your request, we will begin to process your PVG application and contact you for all necessary information, once we have received this from you along with 3 of the following forms of identification we will be ready to submit your application;

• Driving licence

• Passport

• Birth certificate

• Utility bill

• Bank statement

These must be countersigned by an independent person to verify identity.

Payment

We cannot process your application until we have received payment as follows:

• New Applications to join PVG Scheme: £59 (+£15 admin  fee)

• Existing PVG
  – If you answer ‘Yes’ to question A3: £18 (+£15 admin  fee)
  – If you answer ‘No’ to question A3- Scheme Record is required: £59 (+£15 admin  fee)

Your completed application will then be passed to Disclosure Scotland to process.

Once we have received your PVG from Disclosure Scotland, we will forward it to you. As part of the terms of our agreement, you are expected to adhere to our policy relating to the secure handling of disclosure information.
